After you have collected your cookies orders, fill up this portion and detached it from the order form ( the top part's for you to keep. ) , ensure that all money calculations and number of tins are correct. Also, write down your name, class and patrol as shown above. Please put your money in an envelope but do not seal it.

Once again, you're reminded that the order form have to be submitted next Friday, 13 July 2012. Please don't tell us that you forgot to bring and give us all sorts of excuses. Good luck for your sale of cookies (: !
